Healthcare Transition Information for Parents, Caregivers, & Youth
Understand the Differences
As youth transition from pediatric to adult healthcare services, it’s important for caregivers to support their loved ones to understand the differences they will encounter. This presentation will help youth and their caregivers become familiar with the process of engaging with adult providers and how the adult healthcare system works. The presentation will also help youth learn how to prepare for an appointment, what to take to an appointment, and what to expect at the appointment.
